Heath, of the 46 Regiment of OVI, at Van Wert on the 10 day of September, 1861, to serve 3 years; he was born in Miami Co., in the State of Ohio, is 30 years of age, 5 feet 7 inches high, dark complexion, blue eyes, dark hair, and by occupation when enlisted a farmer. During the last two monthsi said soldier has been unfit for duty (blank) days. he (sic) is this day discharged by reason of Etf of surgen & gen order no 36 of 1862

I certify that I have carefully examined the said Benjamin Baltzell of Captain I.N. Alexanders Company, and find him incapable of performing the duties of a soldier because of an ulcerated leg. He has had an indolent ulcer on the region of the tigia for years. It totally unfits him for military duty.

Know ye, That Benjamin Baltzell a corporal of Captain --- Company, (B) Tenth Regiment of Ohio Cavalry VOLUNTEERS who was enrolled on the fourteenth day of Octover one thousand eight nundred and Sixty Two to serve three years or during the war, is hereby Dischareged from the service of the United States, this Tenth day of June 1865 at Camp Chase Ohio by reason of G.O. No. 77 afo dated Washington April 28/65 (No objection to his being re-enlisted is known to exist.*)
Said Benjamin Baltzell was born in Miami Co., in the State of Ohio is 33 years of age, 5 feet 7 inches high, dark complexion, Gray eyes, Dark hair, and by occupation, when enrolled, a Farmer.
Given at Camp Chase Ohio this Tenth day of June 1865.
